Football - how injury and dysfunction affects performance

Flat feet (pes planus) can have a noticeable effect on both performance and long-term injury risk in high-impact, multidirectional field and contact sports such as rugby and football.

These sports require immense stability for tackles and aggressive power for sprinting and sudden changes of direction, making the feet’s role as the foundation paramount.

1. Compromised Stability in Contact and Pivoting
The lack of a natural arch significantly reduces the foot’s ability to act as a stable anchor, which is essential when a player is subjected to external force or needs to execute a sharp turn.

  • Overpronation: Flat feet typically lead to overpronation, where the feet roll excessively inward upon impact or planting. This internal rotation destabilises the entire leg.
  • Reduced Stance Strength: In contact sports (like rugby), this instability compromises the player’s base when setting for a scrum, ruck, tackle, or maintaining balance under heavy physical pressure.
  • Agility and Control: Quick pivots, sidesteps, or sudden braking manoeuvres (like in football) become less controlled and less efficient. The athlete expends more energy correcting their balance after a directional change, resulting in slower recovery rates and reduced overall agility.

2. Less Efficient Force Generation and Explosiveness
Peak performance in these sports relies on the efficient transfer of force from the ground up, whether sprinting, jumping, or pushing off. The collapsed arch compromises this entire process.

  • Loss of the Natural Lever: The foot’s arch should function as a rigid lever and a powerful spring during the push-off phase of running or jumping. When the arch falls, this lever is less rigid, leading to less efficient ground reaction force (GRF) utilisation.
  • Reduced Power Output: This loss of biomechanical efficiency translates directly into:
    – Reduced Acceleration and a slower first step during sprint starts.
    – Decreased Vertical Power for jumping (e.g., winning a lineout in rugby or a header in football).
    – More Energy is lost with every stride or push, taxing the muscles more quickly.

3. Altered Running Mechanics and Endurance
Flat feet can also cause biomechanical adjustments that disrupt an athlete’s natural gait, leading to energy inefficiency, particularly over the duration of a match.

  • Compensatory Alignment: To compensate for the inward roll of the feet, the knees and hips rotate internally. This can lead to inefficient running form, where the energy is absorbed by joints instead of being stored and released by muscles.
  • Endurance Drain: This compensatory effort causes muscles in the calves, shins, and thighs to work harder to stabilise the body. Fatigue builds up faster, leading to a decrease in endurance efficiency and a higher risk of poor form late in games when consistency is vital.

4. Higher Risk of Lower Body Injury
The misalignment caused by flat feet subjects the entire lower limb to abnormal stresses, significantly increasing the risk of specific musculoskeletal injuries.

  • Feet – Plantar fasciits, often due to overstretching of the arch.
  • Ankles – Strain on the inner ankle tendons (e.g., posterior tibial tendinopathy) and increased vulnerability to sprains due to unstable footing.
  • Shins – Greater likelihood of shin splints (medial tibial stress syndrome) from repetitive impact and poor shock absorption.
  • Knees – Patellofemoral pain (pain around the kneecap) and increased strain on the inner (medial) side of the knee ligaments and menisci due to abnormal rotation.
  • Lower back – Compensatory tightness, muscle fatigue, or pain resulting from poor pelvic alignment and reduced shock absorption from the ground.
